Student Athlete Forms

Paris Junior College student athletes should complete and return the following forms:

• Parental Insurance Information Sheet (All Student Athletes)
• Medical Report/Physical (All Student Athletes)
• Housing Application (New Student Athletes - Include Residence Hall Deposit - click here to learn more)
• Online Admission Application (New Student Athletes)

Also include a copy of your high school transcript or transcripts from other colleges. Disregard a form if you have already completed and submitted a copy.
